It's actually a Fitbit watch and you download this face via the Fitbit app. The radiation is low or none if you are meeting your fitness goals. If you don't meet your fitness goals, you show as radiated!!!!
I do have both the Fallout 4 and Fallout 76 pipboy though and have put my phone in them with the app!
